When Something Feels Off — Start Here

A practical guide for diagnosing system, workflow, and data issues.

Most system issues are not purely technical. They are usually caused by missing information, unclear ownership, duplicate records, incorrect statuses, or workflow drift.

Troubleshooting starts by tracing the issue back to the source of truth.

First Troubleshooting Questions

  • Is the item in the Action Log?
  • Does it have an owner?
  • Does it have the correct status?
  • Is the next action clear?
  • Is there a duplicate record?
  • Is the item stuck, aging, or overdue?
  • Does this require escalation?

Common Issue — Missing Work

If work appears to be missing, check whether it was ever logged. Search the Action Log, related workflow views, client records, and communication history.

Fix: Create or update the Action Log entry and link it to the correct workflow or client record.

Common Issue — Duplicate Items

Duplicates usually happen when intake enters the system from multiple places.

Fix: Keep the most complete record, link relevant evidence, and close or archive duplicate references according to system rules.

Common Issue — Incorrect Status

If work is showing in the wrong place, its status or action state may be incorrect.

Fix: Update the status, confirm the owner, and make sure the next action is clear.

Common Issue — AI Confusion

AI confusion usually means the source data is incomplete, contradictory, or unclear.

Fix: Review the source record, clarify the facts, update the Action Log, and escalate if needed.

If something breaks, trace it back to the Action Log.
